As we age, the likelihood of living with multiple health conditions increases. This often leads to taking numerous medications, a situation known as polypharmacy. While these medications are often crucial for managing health, they can also pose risks if not carefully managed. This article explores polypharmacy, its potential impact, and the importance of medication reviews.

What is Polypharmacy?

Polypharmacy refers to the use of multiple medications by an individual, often to manage several coexisting health conditions.  It becomes more common as we age due to the increased prevalence of chronic diseases.

The Balancing Act

While medications play a vital role in maintaining health, taking multiple drugs can sometimes do more harm than good.  The number of medications taken tends to increase significantly after the age of 65. This can lead to several potential problems:

  •   Drug Interactions:  Different medications can interact in unexpected ways, potentially reducing their effectiveness or causing adverse side effects.

  •   Overmedication:  Individuals may continue taking medications long after they are needed, or at dosages that are no longer appropriate.

  •   Difficulty Managing Medications:  Complex medication regimens can be confusing, leading to errors in dosage or timing.

  • Reduced Quality of Life: Side effects from multiple medications can negatively impact overall health and well-being.

The Importance of Medication Reviews

Regular medication reviews are essential for anyone taking multiple medications. These reviews provide an opportunity to:

  •   Identify Potential Drug Interactions: A healthcare professional can assess all medications being taken to identify any potential interactions.

  • Ensure Medications are Still Appropriate:  Medications should be reviewed periodically to ensure they are still effective and necessary.

  •   Simplify Medication Regimens:  If possible, medication regimens can be simplified to make them easier to manage.

  •   Discuss Concerns and Preferences:  Medication reviews offer a chance to discuss any concerns about medications and to express preferences regarding treatment options.

What to Expect During a Structured Medication Review (SMR)

A Structured Medication Review (SMR) is a comprehensive review of all medications being taken, conducted by a healthcare professional such as a doctor or pharmacist.  During an SMR, you can expect to:

  • Discuss your health conditions and medications.

  • Review the purpose of each medication.

  • Identify any potential problems or side effects.

  • Explore options for simplifying your medication regimen.

  • Ask questions and express your concerns.

Preparing for Your SMR

To make the most of your SMR, it's helpful to prepare beforehand:

  • Make a list of all medications you are taking, including prescription drugs, over-the-counter medications, and supplements.

  • Note any side effects you have experienced.

  • Think about any questions or concerns you have about your medications.

  • Consider bringing a family member or caregiver to the appointment for support.

Taking an Active Role in Your Medication Management

Talking to your healthcare provider is a crucial step in improving shared decision-making. This means healthcare that aligns with your personal goals, wishes, and preferences, combined with the clinical expertise of your healthcare team. By actively participating in your medication management, you can improve your overall health and quality of life.
